Discharge process training shows improvements

Thanks to our funding, the division of medicines and therapies have seen a huge improvement in timely discharge of patients once they are medically fit to go home.

Natasha Harmsworth-Blyth, head of discharge operations, saw an opportunity for training amongst the multidisciplinary teams (MDT), to share the importance of each member of the team’s role and responsibilities to deliver timely discharges.

“We created integrated and interactive training sessions to introduce teams to each other and understand why each of them has a role to play in helping to get patients home as soon as possible.

“We focused on the new clinical system, Cerner, and the transition from Epro onto it, teaching colleagues how to use the new system and how important it is to document and update key discharge planning information. Because of this, we’ve also seen much improved data quality and reporting.

“Our position as a Trust across London has repeatedly been seen as one of the best for the number of discharges against the number of patients who are medically ready to go home.

“What’s going well is that everyone in the MDT is working towards one discharge date which is set from the day a patient is admitted, and we all understand who is responsible for doing what they need to do to meet that date and avoid patients having to stay in hospital for longer than they need to."

“We held multiple training sessions across our hospital sites within three weeks and welcomed 300 ward staff, which was incredible. But with approximately 2000 staff in the division, caring for our patients 24 hours a day, seven days a week, we still have a long way to go for everyone to benefit from the training materials and funding invested.

“Every ward who took part in the training will shortly receive a resource pack to refer to, and to share with new staff so they can follow our newly improved consistent processes when they join us.”
