Pharmacy discharge counselling project update

You may remember that we funded a pilot project to support patients who were taking four or five different medications, once they left our hospitals.

Patients were identified on the wards, and the project team contacted them within 48 to 72 hours after they went home. They checked if patients needed further advice about their medication and that they understood how and when to take them to help with their recovery.

Since the pilot launched, 2433 patients have been contacted and 29% of those needed some form of intervention. It might have been that they were not taking the right dose of medication, or a different medication would have been better for them.

We’re delighted that the project won an award for Excellence in Hospital Pharmacy Practice at the Clinical Pharmacy Congress in May has been shortlisted in two categories at HSJ Patient Safety Awards.
